Applications are invited for admission to M.D (Homoeopathy) a three years Post-Graduate Course, 2016-2017 session in Saradha Krishna Homoeopathy Medical College(Self Financing ) at Kulasekaram, Kanyakumari District.

Last date for receipt of filled in application : 25-07-2016 – up to 5.30 p.m.
Applications can be downloaded from the website tnhealth.org and the filled application along with necessary enclosures and 2 self addressed envelopes of 23 x 10 cms size, each stamped to the value of Rs.40/- (Rupees forty only) respectively may be sent to the Secretary, Selection Committee, Commissionerate of Indian Medicine and Homoeopathy, Arignar Anna Government Hospital of Indian Medicine campus, Arumbakkam, Chennai 600 106 before the last date with a Demand Draft for Rs. 3,000/- ( Rupees Three thousand only) which is non refundable towards the cost of application form drawn in favour of the Director of Indian Medicine and Homoeopathy, Chennai 106 from any Nationalised bank, on or after 07-07-2016,payable at Chennai.
PROCEDURE FOR SUBMISSION OF THE FILLED IN THE APPLICATION :
Candidate should send the application in the prescribed form after filling up in his/her own hand writing. Except with regard to those candidates who have studied from 9th Standard Onwards and have undergone undergraduate course (B.H.M.S.) in Tamil Nadu, others seeking admission should furnish Nativity Certificate in the Form annexed.
Applications of service/non-service, private candidate/ Special Category candidates should be sent by registered post/speed post/courier service directly to:

** Service Candidates should send their filled-in application through proper channel with forwarding authority’s remarks.
** Advance copy of application from service candidates will not be considered.
** Candidates must enclose only self attested photocopies of documents.
** It will be the responsibility of the candidates to give correct mailing address on the Intimation slip. If there is any change of address, it should be immediately intimated to the Selection Committee.
** The Selection Committee shall not be held responsible for any loss in transit or for incorrect address given by the candidate.
Documents To Be Enclosed With The Filled In Application :
(i) Recent Photo on first page of Application Form.
(ii) Self Attested copy of Statement of marks obtained from First year to Final BHMS Course.
(iii) Self Attested copy of Internship Certificate.
(iv) Self Attested copy of BHMS Degree certificate.
(v) Self Attested copy of the Medical Registration Certificate.
(vi) Self Attested copy of the Community Certificate from the competent authority indicating the social status of candidates belonging to BC / BCM / MBC / Denotified/ SC/SCA to be produced. ST candidates should produce community certificate issued by Revenue Divisional Officer.
(vii)Two latest conduct certificates in ORIGINAL from Group A officers.
(viii) Identification Slips with two photographs in the spaces provided.
(ix) Two Self Addressed cover with postage stamps of ?.40.00 (one for sending mark sheet and another for sending Hall Ticket) enclosed with Application
(x) Attested Photocopy of Transfer Certificate.
(xi) Acknowledgement / Intimation card with address properly written and affixed with postage stamp of Rs.6.00.
