APPLICATION FORM OF COMMON ENTRANCE TEST CET-2015 For Admission to the MBBS, BDS, BAMS & BHMS COURSES 2015

Admission Procedure :
Introduction :
1. Students will be admitted to MBBS/BDS/ BAMS/BHMS courses of the following constituent colleges of Bharati Vidyapeeth Deemed University on the basis of the merit obtained by them in the All India Common Entrance Test (CET – 2015)

2. CET – 2015 will be conducted on from 1100 hrs. to 1400 hrs. at the examination centres in the following cities : Pune, Navi Mumbai, Hyderabad, New Delhi, Vadodara (Baroda), Indore, Bangalore, Lucknow, Jaipur, Chandigarh, Allahabad, Varanasi and Cochin.The exact location and address of the examination centre of the individual candidate will be communicated to the candidate through his admission card.

3. The CET-2015 will consists of one question paper. It will be set in English medium and will contain 200 multiple choice objective-type questions, 50 each on Physics, Chemistry and 100 question on Biology. These questions will be set in general on the detailed syllabus given on pages 35 onwards. Each correct answer will get one mark. There is no negative marking. Only those candidates who would satisfy or are likely to satisfy the relevant eligibility requirements for admission to a course will be considered eligible to appear for the Common Entrance Test (CET-2015) and subsequently for admission to that course.
i) The candidate seeking admission to any of these courses should be an Indian national and the candidate should complete 17 years of age on or before

Qualifications :
The candidate seeking admission to any of these courses should have passed the Higher Secondary Certificate Examination (H.S.C.) or the Indian School Certificate Examination (ISCE) or any other examination equivalent to 10+2/HSC examination of any recognised Board/ University from any school/college situated in India after 12 years of study.

For admission to MBBS/BDS course the candidate should have passed in the subjects of Physics, Chemistry, Biology/Biotechnology and English individually and must have obtained atleast 50 percent marks in Physics, Chemistry and Biology or Biotechnology taken together in the qualifying examinations 9th May 2015 Eligibility for appearing in CET-2015 and subsequent admission 31st December, 2015.

4. Common Eligibility Requirements for admission to MBBS/BDS/BAMS/BHMS
Nationality and Age : i) Requisite qualifications for admission to MBBS / BDS / BAMS / BHMS courses : ii) For admission to MBBS / BDS course i.e., 10 + 2/HSC or equivalent. The candidate should also have secured atleast 50 percent marks in Physics, Chemistry and Biology taken together at CET-2015 for admission to MBBS/BDS course. The candidate belonging to the scheduled castes/scheduled tribes should have obtained atleast 40 percent marks in Physics, Chemistry and Biology taken together in the qualifying examinations as well as at CET-2015.

The candidate seeking admission to B.A.M.S. course should have qualification as mentioned in 4.2 (i). The candidates seeking admission to this course should have passed 10 + 2/HSC or equivalent examination with Science. The Candidate must have obtained atleast 50 per cent marks in Physics, Chemistry and Biology taken together (in case of SC/ST students 40 per cent)

The candidate seeking admission to B.H.M.S. course should have qualification as mentioned in 4.2 (i). The candidates seeking admission to this course should have passed 10 +2/HSC or equivalent examination in the subjects of Physics, Chemistry, Biology and English individually, and must have obtained atleast 50 per cent marks in Physics, Chemistry and Biology / Biotechnology taken together (in case of SC/ST students 40 per cent)

However, if any seats remain vacant in BHMS course, the candidates having qualifications as prescribed in Central Council of Homeopathy (C.C.H.) as minimum qualification will also be considered eligible for admission. The minimum qualification for admission as prescribed by C.C.H. is “That the candidate seeking admission should have passed 12th std. (HSC or equivalent examination) with Physics, Chemistry and Biology as his subjects.”

v) Candidates who are likely to appear or who have appeared for qualifying examination i.e. 10+2/H.S.C. or equivalent but whose results have not been declared will also be considered provisionally eligible to appear for CET-2015, provided they have offered the above mentioned subjects at the said examinations.

If candidate fails to fulfill the relevant eligibility requirements as mentioned above will not be considered eligible for admission to any of these courses, viz. MBBS/BDS/BAMS /BHMS, even if he/she is placed in the merit list of the CET-2015. The candidates are advised not to submit their application forms for appearing for CET-2015 if they do not fulfill any or all of the relevant eligibility requirements.

vi) If you have passed 10th or 10+02/HSC or its equivalent examination from ANY SCHOOL SITUATED OUTSIDE INDIA, You are NOT ELIGIBLE for admission under Regular or Management Merit Category. If you submit the form under such situation it will be rejected and you will be considered not eligible for final admission to MBBS/BDS/BAMS/BHMS Courses admission even if you appear for the CET-2015.

Important Instructions :
1. Check your eligibility for CET 2015 on bharatividyapeethuniversity.net/
2. You must read information brochure before beginning the online registration process.
3. “*” indicates mandatory field. The boxes marked with this symbol must be filled in or you will not be allowed to complete your registration.
4. Ensure that you have a valid and unique email address before you begin the registration process.
5. You must be ready with recent passport size scanned image of photograph (size < 60 KB) and scanned image of your signature (size < 60 KB) before start registration process.
6. Candidate those who fill the form through Demand Draft (DD) mode need to send application form along with original Demand Draft (DD).Write down candidate name, mobile number behind DD.
7. Candidate those who fill the form through Payment Gateway (Online) mode need not to send application form. Carry application form at the time of examination at test Center.

Estimated time to complete this process: 15 minutes.
Before you begin, you must be ready with all the necessary information that you need to complete the registration process, including your personal information, details of qualifying examination, details of last school attended, DD information(if payment is through DD mode) and scanned image of photograph and signature.

Profile Management :
A. New User Registration
1. Select Course Name for which you are going to apply.
2. Username must be your valid and unique email address.
3. Enter your Profile Password.
4. Primary mobile number is recommended as this number will be used to send important CET related text messages by SMS. Do not add zero/91 before mobile number
4. Read the data privacy policy and indicate your agreement to the policy. Agreement is required to register for the CET 2015.

Click “Submit” to move on to the next section, which is the CET Application.

The Candidate Photograph & Signature :
The Bharati Vidyapeeth Deemed University CET-2015 Applicant Interface requires that copies of your photograph and signature be uploaded as an electronic file at the time of submitting your application. Uploading photographs or signatures that do not meet specifications can result in rejection of the application without any refund of the fee. Photograph Requirements Please pay attention to upload good quality photographs. Poor quality of photographs submitted will lead to rejection of your Bharati Vidyapeeth Deemed University CET-2015 application, without any refund of the application fees. The Bharati Vidyapeeth Deemed University CET-2015 Admit card and Result will be printed with the photograph you submit.

1. The photograph must be in colour and must be recently taken in a professional studio. Photographs taken using a Mobile phone and other self composed portraits are NOT acceptable.
2. Photograph must be taken with White or a very light background.
3. The photograph must have been taken after 1 Nov 2013.
4. Face should occupy about 50% of the area in the photograph, and with a full face view looking into the camera directly.
5. The main features of the face must not be covered with hair, any cloth or any shadow. Forehead, both eyes, nose, cheeks, lip, and chin should be clearly visible.
6. If you normally wear spectacles, glare on glasses is not acceptable in your photo. Glare can be avoided with a slight downward tilt of the glasses for the photo shoot.
7. You must not wear spectacles with dark or tinted glasses, only clear glasses are permitted.
8. Ask your photo studio to provide the image in a JPG format and also on a standard 4.5 cm x 3.5cm print
9. Maximum pixel resolution for JPG: 640×480 (0.3 Mega pixel) (Ask your studio to reduce it to this resolution if it is higher)
10. Minimum pixel resolution for JPG: 320 x 240.
11. The maximum file size is 60 kB (kilo bytes).
12. For your own benefit it may be prudent not to intentionally change your facial features or hair style as in the photograph until the day of the examination.

Signature Specifications :
1. Please put your signature with a black or dark blue ink on a white paper.
2. Get the signature digitally photographed/image scanned by a professional photo studio, and get the image cropped by the studio itself.
3. Only JPG image formats will be accepted.
4. The maximum pixel resolution for the image is 800 x 300.
5. The minimum pixel resolution for the image is 400 x 150.
6. The maximum file size is 60 KB (kilo bytes).
7. Mobile phone photographs of signatures are not acceptable, and can result in rejection of the application without any refund of the fee.

Submit CET Application :
Once you complete the Application and choose the “Submit” button, all your information will be entered and saved into computerized data entry system. This could take several minutes. Please Do NOT close this window while your information is being saved. After your information has been successfully saved, a copy of your application will be available to your login (Account).

Confirmation Page :
If all information has been entered correctly, you will see a confirmation screen. You should read the confirmation screen to make sure all information is correct.

Application Summary :
** Once your CET application has been successfully submitted, logout to your account & login again to download a summary of your application.
** Please save and print a copy of the application for your records.
