Instructions to the candidates to fill the offline application form (Hard copy):
B.Sc. (Sp. & Hg.)/BASLP / M.Sc. (Audiology) / M.Sc. (Speech-Language Pathology)
** The completed application forms will be scanned and processed by a computer.
** The institute will not be responsible for any discrepancies arising due to non-compliance of the instructions.
** Hence, the candidates are advised to strictly follow the instructions given herein.
** Separate application forms will be issued for Diploma Programmes, B.S.Ed (HI), PG Diploma programmes, M.S.Ed. (HI), Doctoral and Post Doctoral Fellowships.
** There shall be no entrance examination for candidates applying under the category of ‘Foreign students’ for B.Sc. (Sp. & Hg.) /BASLP.

General Instructions :
1. Do not fold the application form or make any stray entries on it.

2. a. The application form is common to all candidates seeking admission to various programmes at the All India Institute of Speech and Hearing which have entrance examination conducted by the institute.
b. Separate application forms are available for other programmes for which there is no entrance examination.

3. The application form is issued / available online and the prospectus is available on the institute website. The candidates are instructed to read the contents of prospectus carefully before filling the application form.

4. Candidates should fill the application form using a black ball point pen only. They should use only capital letters while making entries.

5. All entries must be made within the box provided and no part of it should extend beyond the box.

6. Candidates seeking admission for BASLP program at RIMS, Imphal or NSCBMC, Jabalpur may also apply in the application form by selecting the name of the institute they wish to apply (one or more selection is allowed). Payment shall be as follows

7. Candidates seeking admission to BASLP at RIMS, Imphal or NSCBMC, Jabalpur shall send their application form to the Director, AIISH, Mysore. A copy of the printed application form may be send to the respective institute also.

8. Candidates seeking admission in foreign student quota (Candidates who have completed their qualifying examination in a foreign country and /or those holding a foreign passport) shall apply to AIISH Mysore only. Such of those candidates are not required to take the Entrance Examination.

9. Those who have not received their marksheet need not enter the marks. In case results are not declared at the time of application, the marks card may be sent as and when the results are declared, in any case before the AIISH entrance examination.

Item-wise Specific Instructions :
Before filling the application (offline or online) form, the candidates should read item-wise specific instructions given below:
1. Candidates applying for M.Sc. (Audiology) and / or M.Sc. (Speech-Language Pathology) should mention the name of the programme that they wish to apply. In case the candidate wishes to apply for more than one programme, separate application form has to be submitted along with application fee.

2. Name of the candidate: The candidate should enter the name as given in the 10th standard marks sheet. Leave one box empty after each word or initials

3. Father’s Name or Mother’s Name: Father‘s name in full should be filled in the space provided. Mother‘s name in full should be filled in the space provided only if the father is not alive.

4. Application for Institute: Indicate the institute in which you wish to pursue the B.Sc.(Sp&Hg) / BASLP programme. If you wish to apply for more than one institute, separate application form should be submitted along with prescribed fees for each institute.

5. Entrance Examination Centre: Candidates for B.Sc. (Sp. & Hg.) / BASLP should indicate the centre where they would like to appear for the entrance examination. The codes for the different centres are as follows:

A MYSORE
B DELHI
C MUMBAI
D KOLKATA
E CHENNAI
F GUWAHATI
G JABALPUR

Applicants for M.Sc. (Audiology) and M.Sc. (Speech-Language Pathology) will not have any choice for the centre, as the entrance examination for these programmes will be conducted at AIISH, Mysore only.

6. Address: The candidates should write their full postal address in block letters to which all correspondence shall be sent. The information should, as far as possible, include the name of the parent or guardian, house number, number or name of the street, name of the city, name of the taluk / district, email address and phone number. Leave a blank space between each word. Write the name of the State, country you belong to. Write the Postal Pin code.

7. Student’s Date of Birth: Date of birth should be the same as in the 10th standard marks card. It is entered as DAY-MONTH-YEAR.
For example, if your date of birth is 25th December 1989, then enter as
2 5 – 1 2 – 1 9 8 9

8. Student’s qualification: Mention the qualifying examination passed in the space provided. The qualifying examination should be one of the following or their equivalents. DHLS, DHA & ET, II PUC/XII Std, B.Sc. (Sp. & Hg.) /BASLP, or their equivalents.

9. Country in which appeared for qualifying examination: Candidates applying for B.Sc. (Sp. & Hg.) / BASLP should indicate the country where they have appeared for the qualifying examination. For example, if you have appeared for the qualifying examination in India, write India. On the other hand, if you have appeared for the qualifying examination anywhere outside India, then mention the name of the country.

10. Contact number and Mobile number: It is compulsory that a contact number including the telephone and mobile number of the candidate/parent be furnished along with STD / ISD code of the region in the box provided. In instances of non-availability of residential telephone, any other contact mobile number can be provided wherein the candidate/ parents can be contacted. STD / ISD code of the region must be clearly entered. Leave a blank space between STD / ISD code and the telephone/mobile number.

11. Category under which you wish to apply: The codes for different categories are as detailed below. You can opt for more than one category if you are eligible. Put a cross (X) in the appropriate box
A General Merit
B SC
C ST
D North Eastern States
E Foreign
F Diploma (DHLS & DHA & ET)
G Physically Disabled (Ortho)

OBC – Students are eligible to claim seat under OBC category strictly as per Central OBC list and they should produce the certificate in the prescribed Central OBC format.  Format of OBC certificate is enclosed.
For example, if you are applying under General Merit category, then put a cross (X) in the appropriate box A

12. E-mail address: e-mail address of the candidate/parent, should be provided.

13. Nationality: Mention if you are from India or Foreign country

14. Payment details & Transaction id: Details of application fees paid shall be mentioned at the respective spaces provided. The receipt or the Demand Draft UTR / NEFT number along with transanction ID, for fees paid towards the application form (& previous year‘s question paper, if required) should be entered in the space indicated in the application form.

15. Subjects opted and marks obtained

For candidates applying for B.Sc. (Sp. & Hg.) / BASLP:
Subjects opted for B.Sc. (Sp. & Hg.) / BASLP Entrance Exam :
** The candidate applying for B.Sc.(Sp&Hg) / BASLP can write entrance exam in any three of the four (Physics, Chemistry, Biology, and Maths) subjects.
** Subjects studied in PUC/12th: Mention the core subjects studied.
** Enter the marks obtained in PUC/12th.
** The maximum marks and actual marks along with percentage obtained in each subject should be mentioned.

For candidates applying for M.Sc. (Audiology) / M.Sc. (SLP):
Marks obtained in the qualifying examination:
The candidate shall enter the year/semester, total number of papers, maximum marks, actual marks obtained, and percentage marks in each year/semester.

16. Do you require hostel facility? Mention ‘Yes’ if you require hostel facility or ‘No’ if hostel facility is not required.

17. Others :
Affix your recent passport size photograph :
** The candidate should affix a recent passport size colour photograph within the space provided on the offline application form.
** The photo should be pasted within the space provided using gum/glue. Stapler or pin should not be used.
** If the size of the photograph exceeds the margins, it should be trimmed to fit into the space provided.
** If photos are not affixed on the application form and admission ticket in the appropriate places, the application will be rejected.

Declaration :
** After filling the application form, the candidate and one of the parents / guardian should affix their signature in the respective spaces provided in the form.
** Applications without signatures will be rejected.

  2. I want to write the exam in Chennai. I am applying for B.Sc course. Is it possible?

    1. Yes it is possible. In the application form you have to indicate the center where you would like to appear for the entrance examination.
      Entrance Examination Centre: Candidates for B.Sc. (Sp. & Hg.) / BASLP should indicate the centre where they would like to appear for the entrance examination.

    1. Results got declared and counseling schedule get announced.

    1. Those who have not received their marksheet need not enter the marks. In case results are not declared at the time of application, the marks card may be sent as and when the results are declared, in any case before the AIISH entrance examination.

  7. Where can I get syllabus and previous year question papers?
