Instructions For Completion & Submission Of Application Form :
General Instructions :
i) Download the application form (2 pages) and fill the entries. No printed application form is available in the Institute.

ii) Candidates are advised to retain a photocopy of the filled-in application form & DD, before submitting it, for their personal records and future reference.

iii) Application form is to be filled up neatly and legitimately. Please ensure to write address and Signature on the application form only in the space provided. The photograph of the size 3.5cm x 4.5cm must be against a light background.

iv) Information required in the relevant boxes is to be filled in capital letters only and tick (v) clearly the required information in the boxes wherever provided.

v) Option for test centres filled once cannot be changed by the candidate. However, in unavoidable circumstances the Examination centre may be changed by Chairman-Cum-Director.

vi) Please note that name, father’s name and date of birth should be as recorded in the 10th class or equivalent certificate. Any deviation may lead to cancellation of your candidature, whenever noticed.

Specific Instructions :
i. Choice of Centre Codes :
Select two convenient centre Name & codes out of the list of centres given in this brochure and enter them in the order of your preference. Efforts will be made to allot nearest centre, but it may change at the discretion of the Chairman-Cum-Director.

ii. Name of the candidate :
Write your name in capital letters as given in your 10th class or equivalent certificate. Write single letter in a box. Leave one blank box between any two parts of the name as shown in the example below:

iii. Name of Parent/Guardian of the Candidate :
Please write one letter in each box leaving space in between two words, followed by Mr. /Mrs./Dr. as the case may be.

iv. Date of Birth :
Enter the date, month and year of your birth as per the English calendar and as recorded in your 10th class or equivalent certificate. Use numerals 01 to 31 for DATE, 01 to 12 for MONTH and four digits for the YEAR of birth, e.g. April 17, 1985 should be entered as follows:

v. Candidate should fill up the Correspondence Address as well as the Permanent Address. Write your phone number (with STD code),E-mail Id (if any) to which you may be communicated, in the space provided in application form.

vi. If Employed :
If a candidate is employed in any organization, he/she should submit a “NO OBJECTION” certificate from the employer along with the application form.

vii. Nationality :
Please tick (v) in the appropriate box.

viii. Gender :
Please tick (v) in the appropriate box

ix. Caste/Category :
Please tick (v) in the appropriate box
SC/ST/OBC certificate must be obtained in the prescribed Central Govt. format, Caste certificate in state Govt. format is not acceptable.

x. Qualifications :
In the qualifications Column candidates are required to fill up the details of Qualifications i.e 10th pass onwards.

xi. Please mention the date of completion of Internship in the relevant space given in the application form.

xii. Payment details :
The candidate must write the payment details, such as amount of fee, Bank Draft No. & Date in the relevant column of the application form.

xiii. Declaration :
The candidate must sign the declaration given at the end of application form, without which the application will be summarily rejected.

xiv. Photograph of the Candidate. :
Paste (do not staple) three recent (not more than 6 months old), clearly visible passport size photograph (3.5cm x 4.5 cm) on the space provided in the Application form, Admit card and Identity card. It is expected that the candidate will have same appearance at the time of Examination/Counselling as in the photographs.

xv. Signature of the candidate :
Put your usual signature in black ink within the box provided. The signature should be done in the running hand writing and not in capital letters. DO NOT MERELY WRITE YOUR NAME IN CAPITAL LETTERS. This may lead to rejection of your application.
