PROCEDURE TO APPLY :
Prospectus and application form :
** Can be downloaded from Institute website Duly filled in application form can be forwarded to the Institute along with Processing fees & Application form fee/Prospectus fee*.
Can be obtained by registered post: A self-addressed envelope of size 30cms. x 25 cms (A4 size) with postage stamp of Rs. 80 /- affixed on it and Superscribed as “Application for (write name of the course)” should be sent (by speed post preferably,) to the Institute along with Application form fee/Prospectus fee in the form of DD of Rs.150/-*.
** Write (applicant’s) name and course name behind the Demand Draft / Pay Order along with mobile no. of the candidate.
SUBMISSION OF APPLICATION FORM :
** Duly filled application form alongwith admit card (in duplicate), ‘Application form fee/Prospectus fee & Processing fees’ and required self-attested certificates as mentioned in the application form can be submitted in room no. 201, 2nd floor/can be sent by post on Institute’s address so as to reach on or before 30th June, 2015.
** Incomplete application form / application form received after last date of submission, will be rejected and no fees will be returned under any circumstances and no communication in this regard will be entertained.
** Photograph on the application form and on the admit card should be same and latest.
** In case of personal submission and if candidate requires acknowledgment of submission of application form he /she should bring a photocopy of filled application form on which acknowledgement will be provided.
** All correspondence will be done on correspondence address furnished by applicant in application form.
** All payments should be made in the form of Demand Draft/ Pay Order drawn in favor of ‘Director AIIPMR’ payable at Mumbai; Payments in any other form will not be accepted. Candidates are requested to write their name and course name on the back side of the D.D. alongwith the mobile no. of the candidate
** 50% of the fees mentioned in the point no.8.3 (i.e. Rs.6950/-) are to be paid at the time of admission. Remaining 50% are to be paid immediately after cut-off date. (Cut-off date will be declared by University / D.M.E.R., Govt. of Maharashtra).
** Candidates who wish to continue their admission can fill up ‘Status Retention Form’ and can pay full amount of fee at the time of admission in the form of single D.D./ Pay Order (i.e. Rs.13,900/-), however on cancellation of admission after filling up of status retention form entire amount of fee paid will be forfeited.
Cancellation of admission :
** If the student admitted under All India Quota cancels his / her admission before the cutoff date, fifty percent of the total fees will be forfeited ( i.e. Rs. 6,950/-).
** If the student cancels his/her admission after filling up of status retention form or after paying full amount of fees or after last date of admission (i.e. after cut-off date declared by University/ D.M.E.R., Govt. of Maharashtra); full amount of fees paid will be forfeited including deposits for all candidates i.e. admitted under All India Quota as well as under Maharashtra State Quota (Selected by DMER).
** Admission will be cancelled on production of ‘No-Due’ Certificate from the Librarian/ Hostel Warden /Head of the Department along with application.
** Candidate has to apply in prescribed format for refundable deposits immediately after final examination / completion of the course.
Cut- off date for admission will be declared by MUHS, Nashik. D.M.E.R., Government of Maharashtra.
