PGSSET – 2015 PG Super Specialty Entrance Test :

IMPORTANT: Please note that the candidate will not become eligible for attending the entrance test only by ONLINE REGISTRATION. The candidate must ensure that print out of his/her application with all enclosures mentioned in the brochure and original Demand Draft drawn in favor of AMPCK payable at Bangalore reaches the AMPCK office on or before 20/06/2015, 5.00 PM by Post or private courier. AMPCK is not responsible for any postal delay.

ELIGIBILITY :
A person shall be eligible for admission to Post Graduate Super Specialty degree courses only if he/she:
a) Is a Citizen of India.

b) Has a MBBS Degree from a University established by law in India as recognized by the MCI and must possess recognized degree of M.D. / M.S. (or its equivalent recognized degree) in the subject shown against them from a University established by law in India and recognized by the MCI or any other University recognized by the Medical Council of India and declared as equivalent.

a) Has not completed 48 years of age on the date fixed as the last date for submitting the application for the Entrance Test.
b) Has not failed more than twice in the Post Graduate degree examination.

APPLICATION FORM:
(a). Guidelines to fill the online application form :
1. Course applied for: Select from the drop down list.
2. Name of the Candidate: Fill the name as per the Post Graduate DegreeCertificate.
3. Gender : select from the drop down list.
4. Upload Photo: Upload passport size photo as per the specifications mentioned below.
Note: Since the Applicant should upload their photographs in online application entry, the
applicant should scan their passport size photograph (SNAPPED AFTER 01/04/2015 WITH
NAME & DATE) in .jpg format and the file size should be less than 100kb.
5. Date of Birth: The date of birth should as given in the 10th Marks card.
E.g. If the date of birth is 15th January 1965, it should be filled in the15/01/1965 format only.
6. Place of Birth: Enter the place that you belong to
7. Place of Birth – State: Enter the state that you belong to
8. Place of Birth – District: Enter the District that you belong to
9. Nationality: Mention your nationality.
10. Name of the Father: Fill the name of father.
11. Name of the Mother : Fill the name of Mother.
12. Phone: Enter phone Number with STD code.
13. Mobile: Enter Mobile Number.
14. Email: Enter Email address.
15. Post Graduate Qualification: Fill the postgraduate qualification.
15. Duration of course: Enter duration of course.
16. University: Enter University Name where completed the PG Degree.
17. Month & year of Admission: Enter month & year of Admission.
18. Month & year of Passing: Enter Month & year of Passing.
19. No. of attempts taken: Enter No. of attempts taken in qualifying examination.
20. Permanent Medical Council Registration Number: Enter the registration number of State Medical Council and Central Medical Council
21. State Medical Council Registration Number/State: Enter the registration number of State Medical Council and State Central Medical Council

Click on submit button after finishing the filling of application form. Take the printout of the filled-in application and attach two additional recent passport size photographs which is scanned and uploaded. The print out of the filled in application duly signed by the candidate be sent to AMPCK along with the required documents within the stipulated time.

NOTE:
1. Please note that the candidate will not become eligible for attending the entrance test only by online registration. The candidate must ensure that printout of his/her application with all enclosures mentioned in the brochure reaches the AMPCK office. The envelop should be sent addressed to the Chief examiner, AMPCK, Unit No205, Lakshmi Apartments , No. 6, Cornwell Road ,Longford Town, Bangalore- 560025 and sent through speed post or private courier so as to reach on or before 5.00 pm 20/06/2015, .No application would be received in person .The AMPCK does not hold the responsibility for loss or delay in postal delivery.
2. The applications without the attachment of required attested certificates are rejected.

(b) How to submit:
ii) By Post: Print out of Application filled online in all respects along with attested copies of all required certificates may be sent by post or private courier to CHIEF EXAMINER, ASSOCIATION OF MINORITY PROFESSIONAL COLLEGES IN KARNATAKA (Regd.) (A.M.P.C.K), Unit No.205, ‘Lakshmi Apartments’, No. 6, Cornwell Road, Langford Garden, Richmond Town, Bangalore – 560 025 should reach before5-00 P.M on 20/06/2015. Candidates should super scribe on the envelop as “Application for PGSSET – 2015”. The candidates have to download the Admission Ticket for the Entrance Test from the AMPCK website from 24/06/2015.

ENTRANCE TEST FEE : Rs. 7,000/- (Rupees seven thousand only)
Mode of Payment:-The prescribed registration fee should be remitted through Demand Draft only payable at Bangalore in favor of AMPCK
Note: Registration fee is not refundable under any circumstances.

CERTIFICATES TO BE ENCLOSED:
The candidate has to submit the attested photo copies of following documents duly attested by an Gazetted Officer along with the filled application form.
10th Marks card/Birth Certificate for proof of age.
Attempt Certificate of PG Degree issued by the Principal.
Permanent Medical Council Registration Certificate including PG.
UG Degree Certificate.
Marks cards and Degree certificate of Post Graduation.
